The structure of the residency training program at Good Sam has been designed to optimize learning and reduce distraction in both the inpatient and ambulatory settings. Upper level residents alternate call months in hospital medicine or the intensive care setting with non-call months of elective time and continuity clinic . This allows for immersion and deeper learning in all aspects of the internal medicine discipline. While on the wards or ICU, instruction is focused on intensive care, hospital medicine, and consultative medicine. The remaining six months are reserved for elective time in one of the subspecialties; or the selectives such as geriatrics, neurology, or block ambulatory medicine rotation. Embedded within the months of these non-call experiences are two half-day sessions per week of continuity clinic.

CONTINUITY CLINIC

 Beginning in the PGY-1 year and throughout residency, residents assume and maintain care for a panel of patients for whom they are the primary care physician. Most residents are assigned to work with one supervising attending for the duration of their residency training. Interns are typically assigned one half-day per week in continuity clinic during their ward months, and two half-days during their elective months. Upper level residents are paired as "practice partners". Practice partners are responsible for each others' patients while the other is on a call month.

Description of Call Months:

  Team  Schedule 
BGSMC Ward-All 2 interns + 2 seniors + 1 attending 
  • Each team does one week per month of nights (6p-7:45a) with teaching rounds 6:45-7:45.
  • Most other days 7a-6p.
  • When one member of the team is off, the other team members cover.
  • New patients accepted nearly every day.
  • Conferences: Daily rounds, morning report M & W 9:30-10, Th 9-10, grand rounds Friday at 8a.  Cardiology Grandrounds and Tumor Board monthly at noon.
VA Ward  2 interns + 1 senior + 1 attending  
  • Call q 5.  On call, senior and one intern leave by 9p, the other intern stays overnight 4p-8a staffing admissions with the NF resident and nocturnist.
  • Admissions daily except for pre-call.
  • Typical day 6a-5p.
  • Conferences: Daily rounds, morning report M, W, Th 9:30-10, and grand rounds Friday at 11a.
VA Nightfloat- PGY1  2 interns per month
  • 2 interns share all of the nights typically with a 4 nights on, 4 nights off schedule.  No clinics this month but do have assigned reading and attend academic 1/2 day if not post call. 
  • Conferences: No formal schedule but nocturnist developing a curriculum to discuss and present for supervision.
VA Nightfloat- Senior 1 senior (usually PGY3) 
  • Sunday-Friday night 6p-8a supervising the admitting intern and nightfloat intern along with doing some independent admissions along with the nocturnist.
  • Conferences: see above.
BGSMC ICU (GS ICU) 1 intern + 1 senior + weekly teach attending
  • "Under construction".  Currently q4 traditional 24+4 call for the seniors and interns leave at 9pm. 
  • Conferences: Monthly-Sim center central line, codes, ventilator and renal replacement therapy.  Daily- rounds + 1p didactics. 
VA ICU- Senior 4 seniors + 1 fellow + 1 attending

Currently q4 traditional 24+4 call with every post-post day off. 

Conferences: Daily rounds + didactics.
